- Name : XXXX XXXX - Social Security Number : XXXX - Date of Birth : XX/XX/XXXX This complaint is filed under FCRA 605B ( 15 U.S.C. 1681c-2 ), requesting XXXX, XXXX, and XXXX to block information on my credit report that resulted from identity theft and fraud. Specifically, I seek to block the following fraudulent entries : 1. A student loan account opened at US department of education on XX/XX/XXXX, under account number ( XXXX ). 2.A student loan account opened at US department of education XXXX XX/XX/XXXX, under account number ( XXXX ). These accounts have led to several fraudulent hard inquiries and collections on my consumer XXXX credit report. I have attached a list of these inquiries, along with a copy of the consumer XXXX credit report maintained by XXXX, which includes the fraudulent items. I do not recognize any of the accounts, collections, or hard inquiries mentioned above, as they were not initiated by me and are a result of identity theft and fraud. I have also attached a copy of the FTC Identity Theft Criminal Complaint # ( private case number ) as proof that all the aforementioned entries are due to identity theft and fraud.
