I was seen at emergency room due to car accident, XX/XX/2021, XXXX XXXX XXXX, I did provide insurance information to hospital at time of visit, the hospital didn't submit to insurance, and I didn't receive any information from hospital for debt, until I found on credit report, I have requested removal several times, without reply. I asked XXXX to submit payment, and State collection didn't reply with any documentation in the time of compliance, therefore debt is not my responsibility per XXXX XXXX XXXX XXXX XXXX
