The incident happened on XX/XX/2018. I was diagnosed with temporary anxiety but the doctors imstakenly gave me a dose of lorazepam. After taking 2 doses to avoid anxiety, i had to discontinue the medicine to avoid dependence. On the evening of XX/XX/2018, after stopping XXXX - i had an anxiety attack and when I called 911 they told me that the XXXX vehicle was on the way. But my friend also arranged a vehicle to take me to the nearest hospital. But when i was on my old cell phone XXXX number, I clearly told the operator that I do not need the XXXX vehicle but the operator told me that they can not cancel the medical services. I never got any written response for nearly 4 years but now suddenly a collection was added to my credit report by a company called XXXX XXXX services for the amount of {$2200.00} I never utilized the XXXX XXXX and I tried calling the company but they could not give a clear answer and told me that it had gone to collection agency. I strongly deny these charges and never utilized their services and I would kindly request you to mediate and clear this from my credit report. Filed a complaint with MA attorney general office
