An account was fraudulently opened in my name. I immediately ( on XX/XX/XXXX ) went into a branch in XXXX, MN and spoke with XXXX. i was advised that there were aware the account was fraudulent. I called fraud and was advised that they would send me an affidavit to fill out. I have never received it and the account remains open. I need that affidavit.
