On XX/XX/2023 an account was opened in my name for {$12000.00} ( Application Fraud ). The account was with BANK OF AMERICA. The account was closed within 24 hours of being opened. The company advised during the conversation they would honor my request for a copy of the application. I was later informed, I needed to fill out FACT ACT REQUEST FORM. This was sent from a secure fax at a Police Station. I have a confirmed copy of the sent documents which contained ( 4 ) pages. On XX/XX/XXXX, I received a letter stating " we were unable to fullfill your request because we have not received your completed Identity Theft Victim Request form. It appears the Drivers License was not received. I spoke with Supervisor XXXX at Bank of America on XX/XX/XXXX. She review 3 documents that were scanned in and claimed my Driver 's License was not included. I asked to email the confirmation for 4 pages and she declined. I asked to visit a local branch to prove my identify and was declined. XXXX promised a call back on XX/XX/XXXX, with information to the lost copy of my driver 's license. She needed to speak with " Assigned Analysis ''. Today XX/XX/XXXX, I was on hold 83 mins to speak with a supervisor. I was provided a supervisor named XXXX, who within 2 mins disconnected the call. Per the application fraud checklist, I am entitled to a copy of the application. I sent in the required documents from a secured Police Department Fax and have confirmation of the pages received. Bank of America has declined my request per FACT Act Provision 151 to supply the application that was used to obtain a credit card in my name, without my permission. Since they will not properly respond, and now have misplaced a copy of my drivers license, I am very concerned about my information. I do not know the person that opened the account in my name and wish to have a copy of this application- which is why I am reaching out for assistance regarding my request. It appears Bank of America has turned a deaf ear to my concerns regarding my identify theft, so I hope it will be taken more seriously with CFPB. Thank you XXXX XXXX
