Performed a routine, preventative XXXX in XX/XX/2022, at XXXX XXXX. XXXX are done every couple of years as a routine preventative measure for those with XXXX XXXX, including myself. There was no emergency at the time, and this was not an ER visit. Had active medical insurance at the time under XXXX. XXXX terms and conditions advertised 100 % coverage of preventative services. XXXX had also sent me written communication saying the XXXX was covered under my insurance and pre-approved. A few months later, XXXX had only paid a portion of the amount to XXXX XXXX ( rather than 100 % ), and claimed in communications that the service was not preventative, leaving me with a balance of $ XXXX. XXXX XXXX claimed in communications they had billed it as preventative. XXXX XXXX was unwilling to work with me, and referred it to State Collection Service , Inc., a debt collector. I responded to the debt collector disputing the debt, saying it is not my debt, and that they should collect the amount from XXXX. They are still trying to collect from me.
