On XX/XX/2021 I visited XXXX XXXX XXXX located at XXXX XXXX XXXX XXXX, XXXX XXXX, IL XXXX during visit I was advised that I need to do dental cleaning, I had dental insurance and I requested to check and confirm that the procedure will be covered by my dental insurance, XXXX XXXX XXXX representative checked and advised that insurance will cover, and my out of pocket expense will be {$150.00}, I paid the fee immediately with my credit card, signed an agreement, and schedule to perform the procedure on XX/XX/XXXX. On XX/XX/XXXX I came to do the procedure and was told that I need to do deep cleaning, I asked again if it will be covered by my dental insurance, and XXXX XXXX XXXX confirmed that it will, I performed the procedure. On XX/XX/XXXX I received a mail invoice to pay {$700.00}, I called XXXX XXXX and advised that the invoice is incorrect, that insurance should covered, XXXX XXXX XXXX checked, and advised that the procedure that they performed was not the one that is covered by insurance, I told them that I asked to check before they performed it if it will be covered, and they did confirm. If they would have told me that is not covered I would not have done the procedure.
