XX/XX/XXXX - We issued a check for {$11000.00} payable to United States Treasury in payment of quarterly tax ( Tax Year XXXX Form XXXX ) and mailed this check at the post office located at XXXX XXXX XXXX XXXX XXXX, Texas XXXX. XX/XX/XXXX - We received a bill from Dept of the Treasury Internal Revenue Service by mail, notifying us of changes to our XXXX Form XXXX which resulted in taxes owed for {$11000.00}. - We reached out to our CPA for advice regarding IRS letter, raising the issue of why we owed taxes. Our XXXX instructed us to verify the check payment made on XX/XX/XXXX and provide details. - We retrieved the details of the check from Chase Bank online. That's when we became aware of alteration to the check. The check was altered, payee was changed to " XXXX XXXX '', my signature on the check was forged. - We filled out Form XXXX XXXX Power of Attorney and Declaration of Representative XXXX. provided by our CPA to be able to contact IRS in our behalf about the notice and assessed penalties. XX/XX/XXXX -We contacted Chase Claims Department ( Reference Case # XXXX XXXX for follow up. We were informed that our claim was denied due to the time it took us to file this claim. Chase personnel stated we only had 35 days to settle from time of discovery, and since it was filed almost a year later, they no longer have the ability to recover the amount.
