XX/XX/XXXX around XXXX at Sam 's club store on XXXX XXXX in XXXX Colorado. When I was checking out at self check I was approached by an employee from the store and he said that he wants to scan my membership card to verify the address and name is correct. I let him do that he then started clicking on some boxes and answering questions on the screen of a check out. he said he will give me {$30.00} to verify all my information and asked me to input my social security number into the keypad at the checkout. he then proceeded to click on some more things on the screen and then all of a sudden he told me the card is going to come in the mail. I asked him what card and he said it was a credit card. He never asked if I wanted the credit card because I do not want a credit card from that store. He never said the information I'm giving him is going to be used to apply for a credit card so he fraudulently used my information to apply for a Sam 's credit card without telling me or asking me! when I left the store and started processing what has just happened and try to call the store to talk to a manager, a manager was not available. Team lead said the manager will call me back, the manager never called me back. The teamlead said that they know that this is happening and that the employee should not be doing that however they never tried to fix the situation or stop it from happening again!
