I am writing this letter to disclose the services were not rendered to me on or about XX/XX/2021. XXXX XXXX XXXXXXXX deducted a total of {$4400.00} out my account ending in XXXX, when I arrived for my appointment it was canceled. I had a XXXX XXXX completed on XX/XX/2021 per my XXXX request. This procedure required a medical clearance from my physician which she had stated I was in fact cleared. I have attached the letter. Even in goals contract ( see attached ) this condition isnt not a reason to cancel an appointment. Goals have list the reasons to cancel an appointment there for the reason was not listed. I believe my appointment was canceled and my money was stolen and applied to someone else account. This clinic is operated by unregistered employees that are not medical assistants who doesnt have a license with the State of Georgia. I asked to see credentials and was asked to leave. There were unsanitary bags outside of the trash cans. This company is a scammed and I have read when clients appointments have been canceled multiple times and money was stolen and never returned. Bank of America has received my documentation and they will not issue a temporary credit for the fraudulent charges on my account. I am not being treated fairly. My account was drained and I literally dont have anymore money.
