Beginning in XX/XX/XXXX and continuing through XX/XX/XXXX, unauthorized ACH withdrawals were made from my Chase checking account by XXXX XXXX to pay an account that is not in my name. I did not authorize these transactions and did not authorize any third party to use my bank account for this purpose. The unauthorized ACH debits occurred repeatedly over time, all associated with the same ACH originator, and total {$5800.00} across 37 transactions. XXXX XXXX confirmed in writing on XX/XX/XXXX that I am not authorized on the XXXX XXXX account associated with these payments and that the account does not belong to me. Chase later advised that my original fraud packet was lost. To address this, I re-submitted a corrected notarized affidavit by fax and provided XXXX and USPS tracking confirmations. On XX/XX/XXXX, I contacted Chase ACH Fraud. Chase confirmed receipt of my affidavit and documentation but could not provide any determination, reimbursement, or substantive update, stating only that the matter remained under review with an estimated five-business-day timeframe due to the holiday period. As of the date of this complaint, Chase has not resolved the claims, has not reimbursed the unauthorized ACH withdrawals, and has not provided a substantive investigation outcome despite confirmed receipt of all documentation.
