I received 7 emails from US Bank on XX/XX/XXXX between XXXX and XXXX regarding applying for 2 Savings accounts and 1 checking account, approval of those 3 accounts and lastly an alert stating the email on the accounts have changed. I went in person to the US bank at XXXX XXXX XXXXXXXX XXXX XXXXXXXXXXXX CA XXXX a Client relations consultant at the bank who helped me file a fraud claim. She contacted the fraud department via phone, and I went through the process over the phone at the bank. The above accounts were frozen " forced closure '' at approximately XXXX today XX/XX/XXXX. The Fraudster used my name, phone number, DOB, email address, SSN and driver 's license number to open the fraudulent accounts.
