Subject : Formal Complaint Request for Investigation and Deletion of Fraudulent Account from Credit Report To Whom It May Concern, I am writing to formally dispute and request the immediate removal of a fraudulent account from my XXXX credit report. The account in question is : Creditor Name : CITICARDS CBNA XXXX Account Number : XXXX This account appears only on my XXXX report and is not present on my XXXX or XXXX reports. I did not apply for this credit card, nor did I authorize the opening of this account. In accordance with the Fair Credit Reporting Act ( FCRA ), I believe my rights have been violated : 15 U.S.C 1681 Section 602 ( a ) : Affirms every consumers right to privacy with respect to the confidentiality and accuracy of personal financial information. 15 U.S.C 1681b ( Section 604 ( a ) ( 2 ) ) : States that a consumer reporting agency may only furnish a consumer report with written authorization from the consumer. If this can not be provided, I demand that the account be removed from my XXXX report immediately. I am a victim of identity theft. In many cases, the perpetrator makes initial payments to avoid detection, then defaults, resulting in charge-offs that negatively impact my credit history. I have submitted multiple fraud disputes to my bank and can provide supporting documentation upon request. This ongoing issue has caused significant harm to my credit score and financial well-being. I respectfully request that XXXX conduct a thorough reinvestigation of this account, and that Citicards CBNA validate the account with proper documentation, including my signed authorization. If validation can not be provided, I request the immediate deletion of this account from my XXXX credit report in accordance with the FCRA.
