I am writing to dispute the following information that appears on my ChexSystems report from XX/XX/XXXX. Per FCRA 605B ( 15 U.S.C. 1681c-2 ) after identifying the items below as information that resulted from identity theft and providing the required documentation, Chexsystems should block and remove these items no later than 4 business days after receiving the required information. I have attached appropriate proof of my identity, a copy of an identity theft report, the identification of such information related to the identity theft, and a statement by me, the consumer, that the information is not information relating to any transaction by me.
