On or about on XX/XX/year>, company XXXX XXXXXXXX XXXX has violated 15 usc 6802. I did not consent nor have i ever AUTHORIZED to disclose my personal information to nonaffiliated third party. This has caused me significant harm and ive been financial damaged which for reason for this dispute. XXXX XXXX XXXXXXXX can not and will provide any proof to me the consumer with disclosure that such information may be disclosed to such a third party. Finally I the consumer XXXX XXXX was not given the opportunity to direct the institution not to disclose my personal information without my authorization. Finally I am ordeing institutions to remove all negative experiences, that mean late payment, credit inquirys and etc. That is a blatant violation of 15 usc 1681a. Credit utilization is EXCLUDED from the consumer report as well as late payments. AUTHORIZATION TO DISCLOSE WAS NEVER AUTHORIZED. PERMISSSION WAS NEVER GIVEN!!!
