XXXX XXXX XXXX XXXX, IL XXXX Subject : Formal Dispute of XXXX XXXX Account # XXXX and Afni XXXX XXXX Account # XXXX To Whom It May Concern, I am writing to formally dispute the charges and ownership of the XXXX XXXX account XXXX XXXX, which has now been transferred to Afni, XXXX for XXXX XXXX account # XXXX. While I initially authorized the creation of an account, I have not signed any agreement, nor did I receive any of the devices or services associated with this account. Under 15 U.S. Code 1681i, both parties are required to perform a reasonable reinvestigation of disputed information within 30 days. To date, neither company has provided me with evidence of a signed agreement or other documentation proving the validity of the charges. Fair Debt Collection Practices Act ( FDCPA ) Violations : Afni, Inc. continues to collect on this debt without properly validating the ownership or legitimacy of the account. This is a violation of 15 U.S. Code 1692g ( b ), which requires that all collection efforts cease until the debt has been validated.
