Therefore, I am requesting that these fraudulent inquiries be removed from my credit report immediately. Attached to this letter are copies of my credit report highlighting the disputed inquiries, as well as a copy of my government-issued ID for verification purposes. I am also including a signed FTC Identity Theft Report and a police report documenting the fraudulent activity. Please investigate these unauthorized inquiries and remove them from my credit report.
