Consumer Name : ( XXXX XXXX ) XXXX Social Security Number : ( XXXX ) Consumer Date of Birth : ( XX/XX/XXXX ) To whom it may concern, This CFPB complaint has been filed to request pursuant to FCRA XXXX ( 15 U.S.C. 1681c-2 ) that you, the Experian credit reporting agency, block all information appearing on my consumer credit report that is the result of identity theft and fraud within XXXX business days of you receiving this complaint. This fraudulent, inaccurate, and false information consists of a personal credit card account opened at XXXXXXXX XXXX on XX/XX/XXXX, under the account number ( XXXX ), a personal credit card opened at XXXX XXXX XXXX on XX/XX/XXXX, under the account number ( XXXX ), and a personal credit card opened at XXXX XXXX XXXXXXXX XXXX on XX/XX/XXXX, under the account number ( XXXX ). These accounts have resulted in several hard inquiries, collections, accounts being late, along with XXXX charge offs appearing on my consumer XXXX credit report. I have attached a copy of the consumer FICO credit report maintained by you which shows the fraudulent items that were previously mentioned. I am completely unfamiliar with all the previously mentioned accounts, and they are all listed as charged off. None of the accounts, collections, hard inquires, or charge offs were open, made, or initiated by me. All the previously mentioned accounts are the result of identity theft and fraud. I have attached a copy of an FTC Identity Theft Criminal Complaint ( XXXX ) as proof from the XXXX Federal Trade Commission that all the aforementioned consumer credit accounts, delinquency, and payment history are the result of identity theft and fraud.
