THIS ACCOUNT HAS RESULTED IN A COLLECTION APPEARING ON MY CONSUMER XXXX CREDIT REPORT. I DO NOT RECOGNIZE THE AFOREMENTIONED ACCOUNT AS REPORTED BY YOU, WAS NOT OPENED, MADE OR INITIATED BY ME.
