My Personal Sworn Statement & Declaration : I declare under penalty of perjury ( under the laws of the United States. if executed outside of the United States ) that the foregoing is true and correct to the best of my knowledge. Furthermore, I certify that I am permitted by federal and state law to follow this dispute. I also understand that knowing and willful misstatement of omissions of material facts constitutes a federal criminal violation punishable under the 18 U.S.C. 1001. Additionally, these misstatements are punishable as perjury under 18 U.S.C 1621. The items found within my credit report that I listed on the following pages are not related to any transaction that I made. I am alleging that a person or company -without my authorization- used my personal identifying information to apply for goods, services, or money ; and, was successful in creating some accounts. I have filed a formal report with the federal trade commission which I have attached to this letter. In addition, I have also corresponded with the agencies and Creditors listed. The respective agencies have been very cooperative with me and guided me to the point of taking this step to write you directly. I understand that you reserve the right to not delete and block the items. Im requesting you to do so for. I want to make it clear that Im not making this request an error. I have not misrepresented myself with the allegations aforementioned, and I did not obtain any goods, services, or money as a result of the transactions. I attempted to file a police report, but I was informed by my county that it did not constitute an emergency. Furthermore, they informed me that they wouldnt file the report because I didnt have any information on the individual or company who used my information, nor do I know how my information was wrongfully obtained.
