Dear XXXX Customer Service , I am writing to formally dispute a charge and unauthorized account activity that was applied to my name. On XX/XX/year>, a XXXX sales representative opened an account in my name using my social security number and financed phones which I never received or authorized. Account Details : Account Holder Name : XXXX XXXX Account Number : XXXX XXXX Claim Number : XXXX Approximate Transaction Date : from XXXX to XXXX. Financed Amount : XXXX Incident Description : On XX/XX/year>, I visited your store located at XXXX XXXX XXXX, XXXX, NY XXXX with the intent to finance XXXX XXXX 's XXXX XXXX XXXX interacted with a sales representative named XXXX ( XXXX ) XXXX. On XX/XX/year>, my partner and I visited your store located at XXXX XXXX XXXX, XXXX, NY XXXX to finance XXXX XXXXXXXX XXXX XXXX XXXX The sales representative named XXXX ( XXXX ) XXXX encountered a technical issue with her tablet and could not proceed with the transaction. She asked us to return the next day and requested my personal cell phone number to ensure she could assist us next day. We returned the next day, but she encountered the same technical issue. At this point, she had all our potential phones packed for us to take, and I had provided her with my personal information, including my social security number and address. She kept saying the system was acting up and she could not continue with the transaction. As it was the end of the day, we could not keep waiting and left the store to go to XXXX across the streeXXXX. Shortly after leaving, she called me saying she had fixed the technical issue and asked us to return, but by that time, I had already purchased a different phone at XXXX. She sent me messages on my personal number asking for the PIN to deactivate the order, which I provided. I have screenshots of these messages included in the attached file. Despite her assurance, the order was never canceled, leading to the current issue of being placed in collections. This unauthorized activity has severely damaged my credit score, affecting my ability to purchase a car or property. This issue has been ongoing since XX/XX/year>, and despite my efforts, it remains unresolved. I have attached copies Mails received from T mobile highlighting the charges, any relevant receipts, documents to support my claim, and my credit report showing the negative impact. I also attached credit collection mails asking me to pay T-mobile for the services and product i never received. I kindly request an immediate investigation into this matter and a full resolution, including : Closure of the fraudulent account. Rectification of the negative impact on my credit report. Additionally, I demand that my name and account be REMOVED from credit collections immediately. Please confirm once these issues have been resolved. If this matter is not resolved promptly and satisfactorily, I will be forced to consider legal action to protect my rights and rectify the damages caused. I value my relationship with T-Mobile and hope this matter can be resolved promptly and amicably. Thank you for your attention to this matter. I look forward to your prompt response.
