The account with XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX were accounts that were opened fraudulently without my permission or knowledge. XXXX used my identity and had the initial card mailed to my home, which he retrieved without my knowledge, used the account, and made payment for a time without my consent or knowledge. I have completed ( attached ) an ID Theft Affidavit with the FTC. I have also included copies of my government-issued ID and a recent utility bill for address and identity authentication.
