Caine & Weiner Subject : Dispute of Collection Account and Request for Removal from Credit Report To Whom It May Concern, I am writing to formally dispute the collection account placed on my consumer report. The collection agency has not provided the necessary validation information required under 12 CFR 1006.34 ( b ) ( 5 ), and yet, the account has been reported to the credit reporting agencies.
