There is information on my consumer report that is a result of identity theft. I did not give any written permission in accordance with the FCRA to have the below identified transactions furnished on my consumer report. Please block the reporting of this information. The identified transactions are not related to any transaction by me the consumer. This block is not being made in error, or on the basis of a material misrepresentation of fact, I am in fact, requesting this block on behalf of myself. On XX/XX/2024 and on XX/XX/2024 XXXX, XXXX, and Equifax were all notified by certified mail. According to FCRA my rights are in violation of 15 usc 1681c-2 consumer request to block information from alleged identity theft making the credit reporting agency civilly and criminally liable according to 15 usc 1681n and 15 usc 1681o.
