On XX/XX/XXXX, I was looking at my credit report from a credit rating agency. In that report, I noticed an open credit card from an institution that I have never previously transacted with. It appeared that the credit card was successfully opened in XXXX of XXXX. I called the institution to report fraud. They asked some questions regarding my identity ( e.g., name, number, address ) and then said they were going to refer this matter to their fraud department. Just to reiterate, I have never transacted with this institution and I have never held a credit card with this institution.
