Dear XXXX, On XX/XX/XXXX I submitted a request for investigation under FCRA 611 ( a ) to XXXX regarding the following accounts on my credit report : XXXX XXXX XXXX, XXXX XXXXXXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX. In response, I received a letter stating that the information was " VERIFIED ''. However, I am requesting physical documentation to support how this information was verified, as XXXX has been reporting unverifiable and inaccurate information on my credit report, which has damaged my credit score so bad that I cant even get a credit card or a loan in my name. Under federal law, information reporting must be 100 % verifiable and 100 % accurate. As such, I am requesting that XXXX delete this erroneous information from my report. Please do not " update '' or " verify '' this information again without conducting a due diligence investigation. I am requesting that XXXX provide me with written contract and physical documentation supporting the information being reported on my credit report via USPS mail, as screenshots and medical statements will not suffice. THIS DEBT IS NOT MINE, and I require verification with my actual signature on a contract. I value my credit score, and I believe that it is being unjustly degraded as a result of this unfortunate error. I am not requesting a template response to this letter and require specific answers to the questions asked of XXXX. Please note that I intend to pursue legal action if this matter is not resolved. I would like to remind you of Dixon-Rollins v. XXXX Information Solutions , inc. ( 2010 ) and Miller v. Equifax Information Services LLC ( 2013 ), where consumers won their case against the bureaus due to their lack of reasonable investigation of the dispute. I trust that XXXX will do the right thing and investigate this matter thoroughly. If you can not provide proof that the debt is valid, I request that you delete the information from my credit report. I expect a response within 15 days of receipt of this letter. Failure to do so will result in the permanent expunging of the item in question from my credit report. Best regards, XXXX XXXX
