I was charged and billed {$4.00}, dollars, by the insurance company and my employer did not offer me XXXX or send me an election notice, I was not informed about XXXX and did not know how to apply for it. this medical bill was placed on my credit reports with TRANS UNION, XXXX, and XXXX, I challenged it, but the credit bureaus did not remove it, according to Federal Law employers have to provide employees with XXXX notice and election procedures, and I was not presented with notice, and this would make the bill placed on my credit reports wrong and the balances wrong.
