XXXX XXXX XXXX XXXX ( original creditor ) provided no inconspicuous " Opt-out '' or " Right to Rescind '' forms during or after the opening of account XXXXXXXX as required by law in accordance with 15 USC 1635. XXXX, XXXX XXXX XXXX XXXX, and LVNV Funding are in violation of 15 USC 1611 by willfully providing false and/or inaccurate information on my consumer credit report. As a result, I am harmed due to being denied full access to credit, which has created financial hardships for my family and I.
