My name is XXXX XXXX. I A victim of identity theft a long time ago. After that incident there were incorrect and inaccurate items that appeared in my report. Please delete all of the following ACCOUNTS and INQUIRIES from my credit report immediately. Besides these items, also delete the incorrect names and addresses that I did not live in and names I do not go by. They are all a result of ID theft. Please remove these items which are not as soon as possible. Please see attached IDENTITY THEFT REPORT.
