To Whom It May Concern : 15 USC 1681 ( c ) ( 2 ) Except as otherwise provided in this section, a consumer reporting agency shall block the reporting of any information in the file of a consumer that the consumer identifies as information that resulted from an alleged identity theft, not later than 4 business days after the date of receipt by such agency of ( 1 ) appropriate proof of the identity of the consumer ; ( 2 ) a copy of an identity theft report ; ( 3 ) the identification of such information by the consumer; and ( 4 ) a statement by the consumer that the information is not information relating to any transaction by the consumer. I have contacted Equifax fourteen times, via certified mail, consumer protection financial bureaus portal, and Equifaxs dispute portal to dispute fraudulent accounts which were a direct result of identity theft, on my consumer report. Despite providing proof of the Federal Trade Commissions identity theft affidavit, police reports, my current drivers license and utility bill, and a dispute letter stating that the information/ transactions were fraudulent and not relating to any transaction by me, Equifax has failed to follow the law, conduct an actual investigation, and remove all fraudulent information from my consumer report. In accordance with 15 USC 1681 ( c-2 ) ( 1 ) ( 2 ) ( 3 ) ( 4 ), Equifax should have blocked all information resulting from identity theft within 4 days. 15 USC 1681 ( b ) ( a ) ( 2 ) Subject to subsection ( c ), any consumer reporting agency may furnish a consumer report under the following circumstances and no other : In accordance with the written instructions of the consumer to whom it relates. Based on the above laws and circumstantial situation, I demand the deletion of the following account : 1. XXXX XXXX ( XXXX ), Account # : XXXX 2. XXXX, Account # : XXXX 15 USC 1681 ( a ) ( 4 ) There is a need to ensure that consumer reporting agencies exercise their grave responsibilities with fairness, impartiality, and a respect for the consumers right to privacy. As there are a multitude of inaccurate variations, I demand a deletion of all personal identifiable information from my consumer credit report with the exception of : 1. My Full Legal Name : XXXX XXXX XXXX ( delete all other names ) 2. My Address : XXXX XXXX XXXX # XXXX XXXX, VA XXXX ( delete all other addresses ) Please delete all other addresses, as I can not receive mail or any correspondence, nor do I have access to any of these addresses. Please delete these fraudulent addresses and update the address with the correct one above. My only Phone number is ( XXXX ) XXXX XXXX. Delete any and all other phone numbers. I demand my social security number and date of birth be removed from being displayed on the credit file for privacy reasons. Pursuant to the FAIR CREDIT REPORTING ACT 15 USC 1681 ( a ) ( 4 ), I am a federally protected consumer and I have a right to privacy.
