I was in the process of purchasing my first home and an inquiry was made on my credit score on XX/XX/XXXX. On the credit report there was a debt in the amount of {$850.00} for XXXX XXXX that I received at XXXX XXXX XXXX XXXXXXXX XXXX XXXX XXXX in XXXX XXXX, Louisiana on XX/XX/XXXX. I never received a bill for the hospital visit on this date and the debt went to two collection agencies.
