On XX/XX/XXXX, someone attempted to open a XXXX credit card using my name, address, phone number, email and SSN. I did not attempt to open this card and I was contacted by phone by the XXXX fraud department so luckily the application was denied. On the same date, someone successfully opened a Chime Debit Card and a XXXX XXXX XXXX. I successfully canceled and reported the XXXX and Chime card, but was not aware of the XXXX card until I received the card in the mail on XX/XX/XXXX. I have contacted all three credit bureaus but when attempting to obtain my credit report, I received a letter stating that they are not able to provide me a copy because I need to confirm my address. I sent the requested documentation on XX/XX/XXXX.
