On XX/XX/XXXX I went to see XXXX XXXX for annual wellness checkup. After my visit XXXX months later! recieved a bill for {$40.00}. I contacted the Dr office questioned them about the bill. Stating that it was for annual check up. I have XXXX XXXX XXXX and XXXX XXXX insurance. I should not have a copay. Especially for annual check up that is covered at XXXX XXXX. I've seen this XXXX for many years. Often for annual check up. So after another 3 months I received a collection bill from collection agency. I called that agency stating that I have dual insurance and it was supposed to be wellness checkup. The collection agency said they will contacting XXXX XXXX office and will call me if there is issues. Haven't heard back from the collection agency since. So now I schedule for my annual wellness, when I showed up for my appointment, I was told I can't see XXXX XXXX because I didn't pay my bill. As many times I disputed this bill, they told me it's my responsibility to pay for it. I told the office manager that if I truly owed it then I would have paid but it's wrong to bill me for {$40.00} when I have dual insurance and covered XXXX XXXX for wellness checkup. So I called the XXXX XXXX back today XX/XX/XXXX, now they told me that they did not file as a wellness check up but as a new patient. I was not inform about being a new patient especially I has been seeing the XXXX office for many years. This is unethical and insurance fraud.
