Dear Consumer Financial Protection Bureau, I am writing to file a complaint against [ name of credit reporting agency ] for violations of the Fair Credit Reporting Act ( FCRA ), which is codified at 15 USC 1681 et seq. Specifically, I believe that the credit reporting agency has engaged in actions that are in violation of sections of the FCRA, including but not limited to : Section 604, which outlines the permissible purposes for which a credit report may be obtained. Section 611, which requires credit reporting agencies to provide consumers with a free copy of their credit report once every 12 months upon request. Section 611 ( a ) ( 1 ) ( B ), which requires credit reporting agencies to promptly correct or delete inaccurate or incomplete information in a consumer 's credit report. Section 623 ( a ) ( 2 ), which requires credit reporting agencies to conduct a reasonable reinvestigation of disputed information in a consumer 's credit report. Additionally, I want to bring to your attention that the following addresses are non-mailable addresses and should be removed from my credit report immediately : XXXX XXXX XXXX, XXXX XXXX, MI XXXX XXXX XXXX XXXX XXXX XXXX, MI XXXX XXXX XXXX XXXX, XXXX XXXX MI XXXX I have not authorized any person or entity to request or obtain a credit report for these addresses. Therefore, any information related to these addresses is a violation of the FCRA. As a result of these violations, I have suffered damages, which may include but are not limited to : [ describe the specific harm you have suffered, such as lost job opportunities, denial of credit, etc. ]. I am therefore seeking monetary relief for each violation of the FCRA, including actual damages, statutory damages, and, if applicable, punitive damages. I have attempted to resolve this matter directly with the credit reporting agency, but have been unable to do so. I am therefore requesting that the Consumer Financial Protection Bureau investigate this matter and take appropriate action to ensure that the credit reporting agency complies with the FCRA, removes the non-mailable addresses from my credit report, and obtains written consent from me before releasing any credit report information for any future addresses. I have attached documentation supporting my complaint, including copies of my credit reports and any other relevant correspondence with the credit reporting agency.
