I've sent certified letters to Equifax, XXXX XXXX and XXXX ( original creditor ) notifying them that In accordance with 42 USC 290dd-2 ( b ) ( 1 ) ( C ) Im exercising my right to revoke my written consent signed on XX/XX/2017 for your practice to use and disclose my protected health information with any third-party. I have never made a payment to XXXX XXXX in which XXXX uses to collect debt. Ive made any and all payments directly to XXXX XXXX XXXX and will continue to do so until the debt is paid. XXXX XXXX should cease any attempt to collect debt, discard any of my protected health information that may be on file and cease all reporting to the credit reporting agencies immediately. Failure to do so is a violation and will result in me taking legal actions stated in 42 USC 1320d-5 ( c ) ( 1 ) for noncompliance due to willful neglect. As stated above, I will continue to make payments directly to XXXX XXXX XXXX until the balance is paid in full.
