I had a XXXX XXXXXXXX and I had a specific Dr. that did my XXXX there was an incident where I had to have a XXXX, this Dr. which I gave me no right to be seen by me. I requested my Dr. to see me not this Dr- I'm assuming that was on call at the hospital. So I constantly requested from this XXXX XXXX XXXX Assist. proof of paperwork with my signature giving them authorization. Never received anything. I even submitted my paperwork to all three credit bureaus because they also violated the HIPPA law by giving my information to a third party. This has been on my report and I have been fighting this for a while. You are my last hope!
