Here is a summary of my interactions with the company XXXX over the past month. XXXXXXXX XXXX XXXX : XX/XX/XXXX, email sent to XXXX : " To Whom It May Concern : According to your records on XX/XX/XXXX, someone opened up several unauthorized accounts with XXXXXXXX while using my name, address, and personal information. I filed a claim with your claims department on XX/XX/XXXX. The address that these XXXX visa cards are being sent to is my old address and were forwarded to my current address. Today ( XX/XX/XXXX ), I received a second card with a different credit card number than the first one that was sent this past week. I would like to also file a claim against the second card that was sent to me on XX/XX/XXXX. Please stop sending pre-approved XXXX visa cards in my name. Finally, I would like for all accounts and the address on file to be flagged as 'FRAUD ' and 'IDENTITY THEFT '. Also, I filed a police report concerning both incidents. If you have any questions or concerns, please contact me directly.
