In XXXX of XXXX, I mailed an certified letter to this agency requesting that the organization provide me with a legal signed document indicating that I entered into a legal contractual agreement with their organization. On XX/XX/XXXX, I received a medical bill with all of my medical procedures and medications listed on this bill. I did not sign any HIPPA documents giving Americollect authorization to access my medical records. This is a direct violation of my HIPPA rights.
