On XX/XX/2022, I went to a clinic to receive a free advertised assessment of the medical treatment offered by the clinic. I was told my condition qualified, and the treatment was not covered by my insurance, XXXX. Prior to scheduling a treatment their " Patient Release and Wavier of Liability Form '' and Consent to Treatment forms were presented to me, as two separate forms. I read them and took issue with the level of total indemnity they require. I was told I would not get any treatment unless I signed both forms. Then I refused to sign either and requested to speak to an administrator. After an hour waiting at the clinic no one came to speak with me, so I left without signing either form. Copies of the intake forms to register in the clinic, I have in my possession, state I was entitled to a full refund since no treatment was rendered. The next day I started receiving notices from a finance company informing me {$6200.00} was financed in my name at twenty-three percent ( 23 % ) interest and awarded to the clinic. I ignored the emails since I had not consented for any treatment at this clinic and did not receive any treatment at this clinic, thinking it was a clerical error and the emails would soon stop. Emails requesting my response to set up payment arrangements followed daily. On XX/XX/2022, I emailed the finance company 's customer service department and informed them not to make payment to the clinic since I did not consent to any treatment and no treatment was rendered and I had contacted the clinic about their emails. The director of the finance company 's Customer Service Department sent me an email acknowledgement indicating understanding of the situation and they would hold off further credit bureau notifications, if I would make interest payment of {$150.00} a month until the issue was resolved. I have not made any such payments and do not intend to ; because I did not consent to any treatment or receive any treatment. On XX/XX/2022, I sent a XXXX delivered letter to the clinic demanding a refund payment to the finance company and removal of the debt from my name on the major credit bureaus. To date no such action has occurred and my credit rating has plummeted XXXX points.
