On XX/XX/22 I received a request for payment from TRS Recovery Services , Inc for {$120.00} for a purchase made at XXXX XXXX XXXX XXXX paid for by check # XXXX ( Dummy check # ) Drawn from an account at XXXX XXXX XXXX XXXX XXXX XXXX, MO contacted XXXX XXXX XXXX XXXX XXXX XXXX account found with my name. XXXX XXXX verified the check at point of purchase ( unbelievable ). I contacted XXXX XXXX XXXX XXXX XXXX XXXX XXXX investigate the matter at, XXXX ( XXXX ) XXXX, never heard back from them. I contacted TRS Recovery Services XXXX XXXX and received a dispute form which I completed and mailed it back to them. On XX/XX/22 I received a TRS Forgery/Identity Theft Declaration, completed it and mailed it back to them along with all the information I had found to substantiate my claim of fraud/identity theft. The information regarding this matter was given to me by XXXX XXXX, XXXX ( XXXX ) XXXX and XXXX XXXX XXXX XXXX ( XXXX ) XXXX and through my own investigation.
