XXXX XXXX XXXX Inquiry Date : XX/XX/XXXX I did not authorize anyone employed by these companies to make any inquiry and/or view my credit report. In addition, I did not give you written permission or my consent to furnish this information onto my credit report or consumer credit file. This is a violation of the Credit Reporting Act Section 1681b ( c ) which constitutes fraudulent activity and is also a serious breach of my privacy rights. Please delete these fraudulent inquiries immediately from any records that are on your system or validate these inquiries by providing me with copies of any documentation associated with these accounts bearing my signature and/or my written permission authorizing an inquiry, per Federal & State statutes. Furthermore, I also demand you, produce a contract between you and I bearing my signature showing that I gave you permission to furnish any information on my behalf. In the absence of any such documentation bearing my signature, I formally request that these fraudulent inquiries be immediately deleted from the credit file you maintain under my Social Security number. Please note that you have 30 days to produce the requested contract and to complete this investigation, as per the Fair Credit Reporting Act section 611.
