It was for a medical bill that I had with XXXX XXXX XXXX XXXX XXXX, XXXX XXXX XXXX, XXXX XXXX, XXXX, NV XXXX, Account # XXXX, for {$790.00} in XX/XX/XXXX. I have never received anything else from them the insurance said they had to send some documents in before they paid and that is the last I have heard from them.
