Statement received from C $ I collection agency, stating I owed {$340.00} to XXXX XXXX XXXX XXXX Texas. I immediately sent a debt validation letter to the collection action on XX/XX/2022 and received a response letter dated XX/XX/2022. Inside the letter I received on XX/XX/2022, is nothing but a report from XXXX XXXX XXXX XXXX Texas, with a medical imaging name and procedure code and date and amount. This is on no office letter head or anything verifying XXXX department, this could be the Debt collector falsifying this paper for all I know. There is no other information showing me where this is my actual date, no signatures where I had this procedure, and any proof where I released this Credit agency to have ACCESS to my MEDICAL HISTORY or RECORDS, which this is in violation of HIIPAA. This is not my debt they say I owe, and I need this issue resolved
