A person pretending to be me has reported to XXXX, XXXX, and XXXX that my credit cards have been missing/lost and requested new credit cards to be sent to my address. They also submitted a " change of address '' request with the USPS which i never authorized or knew about. They also opened a new credit card/account with XXXX XXXX. I have called XXXX, XXXX, XXXX, and jet blue and had all my cards/accounts closed/replaced. The dates of these events take place between XX/XX/XXXX and XXXX/XXXX/XXXX. I also filed a police report describing the events on XX/XX/XXXX with the XXXX XXXX police department in OH.
