There is an inaccurate account listed on my credit report that I have requested documentation for multiple times but I have failed to receive documents via mail/fax/e-mail. The amount the account was placed into what appears to be a Debt Collection Agency titled " XXXX '' with account # XXXX was opened in the amount of {$120.00} on XX/XX/XXXX. The account in reference has been in collections since XX/XX/XXXX in a state which I have not resided in since XXXX. I have failed to receive any type of documentation from the credit reporting agency as well as the creditor/original creditors in regards to the validation of this debt. I request a formal investigation into this matter and documentation mailed to me showing the original account opening including photocopies of driver 's license at the time of opening, signatures, and other vital information required to open an account.
