Regarding : report number XXXX I am a victim of identity theft. I am/have formally disputed an account ( s ) that I have learned has been made, opened or applied for without my consent and your company is now reporting such inaccurate information. I did not make this transaction or open or apply for this account and have not authorized anyone else to do so for me. In my original dispute date XX/XX/XXXX, I asked your company to investigate AND furnish me with your findings. I asked that the account be removed if you found no legal proof the account belonged to me and if the account did belong to me, I asked that your company furnish me with the full name and contact for the person who verified the account as accurate as well as a copy the agreement with obtained my legal wet signature showing that I authorized the account ( s ). Your company has done neither! Your company has provided me with NO proof your investigation! In fact, your company responded to my request by sending a computerized response to my OLD address, an address I have clearly giving your proof I no longer resided at, during my first dispute on XX/XX/XXXX.
