It appears that on XX/XX/XXXX credit of {$1000.00} was extended fraudulently and a purchase for that same amount was made at XXXX. On XX/XX/XXXX I received a card in the mail which I did not apply. That night I notified the card agency and they indicated that they would send an affidavit for me to complete and return. The affidavit was dated XX/XX/XXXX although I did not receive it until later in the month. I filled it out and returned it with a copy of the police report. This was mailed certified with a return receipt on XXXX and returned within the 20 days as requested. As of today XXXX the account has returned to my credit reports and I have submitted the police report to the credit bureaus and will once again issue a dispute regarding this account.
