FTC Report Number XXXX XXXX was informed of 5 unauthorized inquiries on my credit file resulting from Identity Theft on XX/XX/XXXX. Article Number : XXXX XXXX XXXX XXXX XXXX The inquiries that were unauthorized were : XXXX on XX/XX/XXXX XXXX XXXX on XX/XX/XXXX XXXX XXXX XXXX XXXXXX/XX/XXXX XXXX on XX/XX/XXXX XXXX XXXX XXXX on XX/XX/XXXX I sent an affidavit of truth citing FCRA violations along with an Identity theft affidavit and an invoice for {$5000.00} due upon receipt on XX/XX/XXXX for violating my rights under the FCRA.
