XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX : XXXX NY XXXX Last 4 Digits of SSN : XXXX While looking over my yearly credit report I see that there is some incorrect information regarding my personal information. There are addresses reporting to my credit report that are not associated with me. It seems as though you or the creditors have mixed my file with someone else that is in my family which is understandable being that our names are similar. I have included my license and proof of address to assist with the handling of this dispute. I am asking that you please remove any and all addresses that do not reflect the address that is on my license and/or proof of address so that the information on my credit report is accurate. Thank you so much for your time in assisting me with this matter. Please send me correspondence that shows that these changes have been implemented.
