This has been reported on my XXXX for yrs as unpaid or paid, switching back and forth. The name of the company is XXXX XXXX XXXX XXXX but that changes all the time as well, or doesnt even have any info or name. I have disputed this account many times and asked for a debt verification proving this is my debt. I have never received any communication or response back. I believe this account is in violation of the HIPAA act of 1996 and any information they have received has been without my consent. That is why they have failed to respond back to all of my requests for proof this is mine!
